UNIT 5 PARENT VOLUNTEER SCREENINGS
RENEWAL FOR PARENTS WHO HAVE COMPLETED VOLUNTEER SCREENING IN PAST YEARS
Parents who were approved through the online background screening process in 2009-2010 or 2010-2011 do not need to complete the online process in 2011-2012. Instead, please complete and return this Renewal Form to the band office.
SCREENING FOR PARENTS WHO HAVE NOT PREVIOUSLY COMPLETED THE ONLINE SCREENING
Opportunities to complete the screening will be given throughout the year, including during school registration.
FOR STATE FARM ASSOCIATES
Any State Farm Associate who volunteers 40 or more hours with the Normal West Bands is eligible to apply for a Good Neighbor Grant through the State Farm Companies Foundation. Through the Good Neighbor Grant Program, the State Farm Companies Foundation awards a $500 grant on behalf of each eligible associate (active employee, agent, or retiree) who volunteers a minimum of 40 hours to an eligible nonprofit organization or school. Grant applications may be submitted every six months.
When completing an application, please list "McLean County Unit District 5 Music Parents Association" as the organization and "Normal West High School Bands" as the purpose. (Note: we intentionally omit "Community" from our name to avoid confusion with NCHS.) The recipient ID is "7017," and the organization's address is in Bloomington. Online submissions will be sent to the Music Parents Treasurer for authorization. Printed copies can be submitted to the band office.
Once you have submitted a grant application, we kindly ask that you inform the band directors so we can account for all grants.

We are very excited about the many opportunities that will be offered to band students this year! Simply put, we cannot offer all of these experiences without parental involvement. We understand that our families have a variety of talents, interests, and availability. The directors and Band Parents’ Organization are confident that there are opportunities suited for each parent! We ask that you become involved in one way or another—the experiences are rewarding, the company is good, and it is an opportunity to watch the students accomplish great things! Even the smallest task makes a big difference!
